# Obagi

The first manufactured drug in this register rather than a compounded blend, and it reads like one: pharmacopoeial concentration, full excipient list, a real package insert. What it does not say is that the FDA has never found it safe and effective, or what ochronosis is.

## Verdict

One product assessed, so this is a note rather than a pattern analysis. But it is a useful control for everything else here, because Obagi is not a telehealth compounder — it manufactures defined drug products, sells them through physicians, and labels them the way drugs are labelled. Hydroquinone USP 4%, restated as 40 mg per gram, with every excipient printed and a full Prescribing Information linked. Contra-indications, warnings, precautions, pregnancy category, nursing and paediatric sections all sit on the product page rather than behind a click.

The formulation is the best of its kind we have assessed. Hydroquinone oxidises, and this one carries a published three-part antioxidant system — ascorbic acid, sodium metabisulfite, BHT — with lactic acid holding the pH. It also carries the clearest sun-protection sentence in the register, giving the instruction and the mechanism together, and an unusually candid warning that its own metabisulfite can cause anaphylactic and asthmatic reactions.

Two things are missing and both are specific to the drug. Exogenous ochronosis — hydroquinone's defining long-term risk and the reason it is a course rather than a maintenance product — appears nowhere on the page or in the label. And the page never states that Obagi Nu-Derm Clear is an FDA-unapproved drug, though the FDA's own listing for it carries exactly that disclaimer. 'RX ONLY' plus a linked insert reads as an approved medicine to anyone who has not checked.

The linked insert is also sixteen years old, marked Rev. 07/10, and its ingredient list for this product disagrees with the one printed on the page above it. Two documents, same company, same product, different formulas — and no way for a reader to tell which is current.

## What the reviewed offering tells us

### 01 — It labels like a drug because it is one

Active stated as hydroquinone USP 4% and again as 40 mg/g, full inactive list on the page, and contra-indications, warnings, precautions, pregnancy category, nursing and paediatric sections printed rather than linked. A complete Prescribing Information PDF sits alongside. No compounded product in this register comes close, and the difference is structural rather than a matter of effort.

### 02 — The regulatory status is the one thing it will not tell you

Obagi Nu-Derm Clear is a marketed unapproved drug — legal, prescribed for decades, carrying an NDC, never taken through a New Drug Application. The FDA's own listing states it 'has not been found by FDA to be safe and effective, and this labeling has not been approved by FDA'. The product page says none of this, and 'RX ONLY' beside a full insert implies the opposite.

### 03 — No ochronosis, and a stopping rule about the wrong thing

Neither the page nor the full label mentions exogenous ochronosis, the paradoxical darkening that makes hydroquinone a course rather than a maintenance product. The label's only stopping rule is a futility rule — discontinue if no improvement at 8–12 weeks — which addresses what to do when the drug fails, not when to stop if it works.

### 04 — The label and the page disagree about the formula

The page's inactive list for Clear includes phenyl trimethicone, PPG-2 myristyl ether propionate and TEA-stearyl alcohol. The linked label's list for Clear has none of them and includes stearyl alcohol instead; two of the three appear under Blender, a different product in the range. The likely explanation is a reformulation the 2010 insert never caught up with, which is still two contradictory documents published side by side.

### 05 — Everything is computable except the cost

The fill size is published in the label — 2 oz, 57 g — and the dosing is stated as a thin application twice daily, which is more than almost anything else here provides. There is no price anywhere on the page and nothing can be bought from it; buyers are directed to find a physician. Cost per application is the one figure that stays out of reach.

## Credit where it is due

- States its active as hydroquinone USP 4% and as 40 mg per gram — pharmacopoeial precision no compounded product here matches.
- Publishes the full inactive ingredient list on the product page.
- A published antioxidant system appropriate to hydroquinone: ascorbic acid, sodium metabisulfite and BHT.
- Complete Important Safety Information on the page, including pregnancy category C, nursing mothers and paediatric use.
- The best sun-protection sentence in the register — instruction and mechanism in one line.
- Discloses that its own sodium metabisulfite can cause anaphylactic symptoms and asthmatic episodes.
- Links a complete Prescribing Information PDF, including fill size, NDC, dosing and storage.
- Instructs a 24-hour patch test before first use — the only product here that does.
- A genuine physician gate rather than an online questionnaire, which for hydroquinone is the correct level of oversight.
