# L-17 — Adult acne prescription combinations: what the ingredients change

A fixed combination, a custom formula and separate prescriptions create different tradeoffs. Compare the antibiotic, retinoid, vehicle and follow-up plan before counting how many actives fit in a pump.

An acne treatment can combine mechanisms without making every combination equivalent. A finished product has a defined ratio and vehicle; a compounded prescription varies by patient; separate prescriptions allow the clinician to change one component at a time. Those distinctions are more useful than assuming that three ingredients must outperform two.

## Two approved combinations, different ingredients

| Field | Value |
| --- | --- |
| CABTREO | Clindamycin phosphate 1.2% / adapalene 0.15% / benzoyl peroxide 3.1%; includes an antibiotic. |
| TWYNEO | Tretinoin 0.1% / benzoyl peroxide 3%; separately encapsulated, without an antibiotic. |

Both labels describe acne indications. Their vehicle-controlled trials do not establish which product is better for a particular adult or prove a head-to-head advantage. They also do not establish cosmetic-aging benefits simply because they contain retinoids. Review the actual trial population and endpoint with the treatment goal in mind.

## Why the antibiotic question matters

AAD guidance recommends benzoyl peroxide with topical or oral antibiotics to reduce resistance. If clindamycin appears in a proposed custom formula, ask how the complete regimen follows that principle and when the antibiotic will be reassessed. Do not infer that every formula contains clindamycin, and do not add a second treatment without checking the existing prescription.

## Compatibility depends on what was formulated

TWYNEO is a concrete example of tretinoin and benzoyl peroxide in an approved finished cream. It does not validate mixing any two separate tubes. Ask the prescriber or pharmacist about the named products and their timing. A blanket never-together rule and an equally broad always-compatible rule both lose the formulation detail.

## Questions for an adult with an existing routine

- Which current prescription or acne product should be replaced rather than duplicated?
- What improvement should trigger continuing, changing or simplifying the regimen?
- Who should be contacted if irritation or another adverse effect appears?
- Would separate prescriptions make it easier to change one component?
- How do the usable period, quantity and pharmacy price affect the total cost?

For painful, scarring or persistent acne, the discussion may need options beyond another topical combination. A review site can explain labels and evidence limits; it cannot decide the appropriate escalation from a product list. Take the comparison to the clinician with photographs, prior treatment history and the actual products you use.
