# L-11 — Before a compounded melasma prescription: five questions to ask

Diagnosis, exact ingredients, treatment area, duration and follow-up belong in the plan. A four-ingredient sales page cannot supply them for every patient.

A melasma cream can contain several medicines and still leave a reader unsure what will be prescribed. The AAD describes hydroquinone and retinoid/corticosteroid combinations among treatment options, alongside sun protection and other approaches. That establishes a treatment family to discuss; it does not settle a patient's recipe or course.

## 1. Has the pigmentation been identified?

Ask the clinician what supports the diagnosis and whether an examination in person is needed. Dark spots and uneven tone are descriptions, not a guarantee that the same treatment applies. A comparison of melasma creams is most useful after the condition and treatment goal have been established.

## 2. What is the exact formulation?

Request every active ingredient, concentration and the intended vehicle. Strut's product-specific melasma section names hydroquinone, tretinoin, hydrocortisone and kojic acid. The retrieved page also contains unrelated eye and neck sections. Their percentages cannot be used to complete a missing melasma prescription.

Our Tri-Luma review provides a label-based reference for a different finished combination. It should not be treated as proof of equivalence: changing a component, concentration or base changes the preparation being evaluated. Nor should a higher hydroquinone number be interpreted as automatically more effective or appropriate.

**COMPARE THE ACTUAL FORMULAS**

- [R-35 Galderma Tri-Luma](/reviews/tri-luma-cream-review) — RECOMMENDED 85/100 — A specific approved finished combination, not a universal template.
- [R-16 Agency Dark Spot Rx](/reviews/agency-dark-spot-formula-review) — CONDITIONAL 75/100 — An existing assessment of another pigment-treatment offering.
- [R-20 Dermatica Melasma Treatment](/reviews/dermatica-melasma-review) — CONDITIONAL 69/100 — Read the service's own formula and course disclosures.

## 3. Where and how long is it meant to be used?

Ask for a defined treatment area and a duration or reassessment point. Strut calls its blend a short-term treatment while advertising auto-refill. The prescription and dispensing arrangement should explain how those terms work together. A billing interval cannot independently determine how long to use a medicine.

The same applies to quantity. A larger bottle does not authorize treating a larger area, and a remaining supply does not settle whether to continue. Have the clinician explain what should happen at the review point and how to handle the next scheduled shipment if the plan changes.

## 4. What changes the plan?

Discuss pregnancy plans, breastfeeding, other prescriptions and previous reactions. The AAD advises avoiding hydroquinone and retinoids during pregnancy. The relevant decision is whether the whole proposed plan suits the patient's circumstances, not whether a website presents it as a cosmetic routine.

Ask which reactions require contacting the clinician and what to do if pigmentation worsens or the diagnosis becomes uncertain. This article does not assign a universal course length or dosing schedule. Those should come from the clinician who can evaluate the particular prescription and patient.

## 5. Who follows up, and what happens after the course?

Obtain the pharmacy name, storage instructions and beyond-use date, plus a route to the prescriber. Ask how follow-up, maintenance and sun protection fit together after the initial treatment. A prescription is easier to evaluate when it describes the next decision as well as the first application.

## A current example of why the course must be individual

Agency's Dark Spot Rx page now describes hydroquinone and non-hydroquinone possibilities with clinician-directed transitions. The public offer does not establish one fixed calendar for every patient. Request the current prescription, the planned review point and the reason for any transition rather than copying an old article's cycle.

Also record the mass, actual charge and any change in the next formula's price. A pigment-treatment course can span more than one preparation. Comparing only the first shipment may miss the care and products the clinician expects to follow it.
