# L-18 — Prescription skincare follow-up: what to establish before the first refill

A visit, a messaging window, a refill authorization and a recurring shipment are different things. Put them on one timeline before comparing services or deciding that care is included.

The first shipment is an easy milestone to compare. The harder question is what happens when treatment needs to change. A service can provide a month of messages, a finite prescription, automatic dispensing and a separate paid reassessment, each with a different clock. Record them separately so that a routine clinical question does not arrive after the access you expected has ended.

## Four dates to write down

| Field | Value |
| --- | --- |
| CLINICAL REVIEW | When the clinician wants to assess progress, side effects and continued need. |
| MESSAGING WINDOW | When included follow-up questions end and how later questions are handled. |
| PRESCRIPTION / REFILLS | How much was authorized and whether another assessment is needed. |
| NEXT SHIPMENT / CHARGE | When dispensing and billing recur, and how changes or pauses work. |

## Published windows are useful, within their scope

MDLIVE lists 30 days of post-visit messaging, one condition per dermatology visit and plan-dependent fees. DocBright and DermatologistOnCall also publish defined follow-up terms in their reviewed offers. Read each source's scope: a question window does not by itself promise another free appointment or an unlimited sequence of new prescriptions.

**COMPARE THE FOLLOW-UP TERMS**

- [R-71 MDLIVE Online Dermatology](/reviews/mdlive-dermatology-review) — CONDITIONAL 85/100 — Plan-dependent visit with a stated messaging period.
- [R-60 DocBright Online Dermatology](/reviews/docbright-online-dermatology-review) — CONDITIONAL 85/100 — Published cash fees and reassessment requirements.
- [R-61 DermatologistOnCall Online Dermatology Visit](/reviews/dermatologistoncall-online-visit-review) — CONDITIONAL 80/100 — One-condition scope and post-visit questions.
- [R-62 SkyMD Online Dermatology](/reviews/skymd-online-dermatology-review) — CONDITIONAL 80/100 — A different messaging period and individual coverage estimate.

## Ask how the service handles change

Before starting, describe an ordinary scenario: the product irritates your skin, the prescribed amount lasts longer than expected, or improvement is incomplete at the review point. Ask who responds, how quickly you should seek clinical advice, whether that contact costs extra and how a change affects the pharmacy order. The aim is an explicit plan, not a guarantee that treatment will never need adjustment.

## Different ingredients can need different review questions

For an antibiotic-containing acne regimen, ask about duration and resistance prevention. For a hydroquinone or corticosteroid-containing pigment formula, ask when the course will be reassessed or transitioned. For a retinoid, ask how tolerance and the treatment goal will be reviewed. A universal refill schedule cannot answer all three discussions.

- [R-16 Agency Dark Spot Rx](/reviews/agency-dark-spot-formula-review) — CONDITIONAL 75/100 — An offering whose formulas may change under clinician direction.

## Prepare a record you can carry between services

- Keep the exact medicine name, strength, base if known and pharmacy label.
- Record the start date, actual use and any change directed by the clinician.
- Keep photographs under comparable lighting if the clinician requests them.
- Save relevant visit notes and the route for a question about an adverse effect.
- Check what happens if an in-person examination or procedure is needed.

An automatic renewal is a purchasing mechanism. Clinical continuation should follow the treatment plan. If the two timelines no longer match, contact the care team and the dispenser so the next shipment reflects the current prescription rather than an outdated routine.
