# R-53 — Almirall Seysara — Sarecycline 60 / 100 / 150 mg

A tetracycline engineered narrow, dosed by body weight, with a label that says efficacy beyond twelve weeks has not been established and that it must not be used for infection. The second considered answer in this register to the antibiotic problem — and the effect size is 22%.

R-48 showed one way to prescribe a tetracycline responsibly: drop the dose below the antibacterial threshold so it stops being an antibiotic at all. Seysara is the other way. It is genuinely antibacterial, but engineered with a narrower spectrum than doxycycline or minocycline — less activity against the gut flora that broad-spectrum tetracyclines disturb — and dosed once daily by body weight rather than by convention.

The limitations of use do the same work Oracea's do. Efficacy beyond twelve weeks has not been established. Safety beyond twelve months has not been established. It has not been evaluated in the treatment of infections. And the label states directly that to reduce the development of drug-resistant bacteria and maintain the effectiveness of other antibacterial drugs, it should be used only as indicated. Four sentences, each one narrowing its own market.

The efficacy is modest and honestly reported. Across two twelve-week placebo-controlled trials in 2,002 subjects, IGA success reached 21.9% and 22.6% against 10.5% and 15.3% on placebo. Inflammatory lesions fell 52.2% and 50.8% against 35.2% and 36.4%. That is a real drug producing a real but unspectacular effect — roughly one subject in five reaching clear or almost clear.

Two things pull it to conditional. The price is around $262 for thirty tablets with a discount card, against an average retail near $1,322, for a twelve-week course of a tetracycline when generic doxycycline costs a fraction of that and no head-to-head has ever been run. And it is still an oral antibiotic: the narrower spectrum reduces the stewardship problem, it does not remove it, and the twelve-week limit is on the label because nobody has evidence for anything longer.

## Actives, as published

| Active | Concentration | Role | Benchmark | Assessment | Flagged |
| --- | --- | --- | --- | --- | --- |
| Sarecycline | 60 / 100 / 150 mg | Narrow-spectrum tetracycline-class antibacterial, dosed once daily by body weight | Doxycycline at 50–200 mg and minocycline at 50–100 mg are the broad-spectrum comparators | Designed rather than repurposed: a tetracycline with deliberately reduced activity against enteric Gram-negative organisms, which is the part of the spectrum that causes the collateral damage. Weight-banded dosing is the other unusual piece — three tablet strengths so the dose matches the patient rather than the habit. Neither feature has been tested against an older tetracycline in a trial. | yes |

## Scoring

| Criterion | Weight | Score | Note |
| --- | --- | --- | --- |
| EVIDENCE FOR THE ACTIVE | 30% | 74/100 | Two twelve-week multicentre randomised double-blind placebo-controlled trials in 2,002 subjects with prespecified co-primary endpoints, both met, and unusually good representation — 51% adults, 15% Black or African American. Marked down because the effect size is modest, the two trials differ on the placebo arm by five points, and no comparison against an existing tetracycline exists. |
| FORMULATION INTEGRITY | 20% | 76/100 | Three tablet strengths supporting weight-banded once-daily dosing, which is a real improvement on the one-size convention for oral acne antibiotics. The narrow spectrum is a molecular design decision aimed squarely at the stewardship problem. Marked down because neither the spectrum advantage nor the weight banding has been demonstrated to change an outcome in a trial. |
| DISCLOSURE | 15% | 92/100 | A limitations-of-use section stating that efficacy beyond twelve weeks is not established, safety beyond twelve months is not established, the drug has not been evaluated for infections, and that it should be used only as indicated to limit resistance. Both trials published with registration numbers, full demographics, and the placebo arms in plain sight. Held back only because no comparator drug appears anywhere. |
| TOLERANCE IN USE | 15% | not scorable | Not assessed. Tetracycline-class effects — photosensitivity, oesophageal irritation, gastrointestinal upset, and vulvovaginal candidiasis in women — are the practical questions for a twelve-week oral course and a panel would be needed to place this against doxycycline. |
| TEXTURE & WEAR | 10% | not scorable | Not applicable — an oral tablet. Left unscored rather than credited, since the criterion cannot discriminate. |
| COST PER APPLICATION | 10% | 40/100 | One tablet daily, thirty to a pack, so cost per dose is exact. Around $262 discounted and $1,322 at average retail for a twelve-week course, with no generic, against generic doxycycline at a small fraction of that. The stewardship argument is real; whether it is worth this multiple has never been tested. |

_A criterion that cannot be assessed from published information is not guessed at. Its weight is removed from the denominator, and the coverage figure above says how much of the model survived._

## Strengths

- A limitations-of-use section that states four separate things the drug has not been shown to do.
- States that efficacy beyond twelve weeks has not been established — an explicit stop point.
- States that safety beyond twelve months has not been established.
- States that it has not been evaluated in the treatment of infections.
- Carries a direct resistance-stewardship instruction in the indication itself.
- Narrower antibacterial spectrum than the tetracyclines it replaces.
- Weight-banded dosing across three tablet strengths.
- 2,002 subjects across two placebo-controlled trials, both registered.
- 51% of subjects were adults and 15% were Black or African American — better representation than most of this register.
- Once daily, which is easier to sustain than twice.

## Weaknesses

- Effect size is modest: roughly one subject in five reached clear or almost clear.
- Around $262 to $1,322 for thirty tablets, with no generic.
- No head-to-head against doxycycline or minocycline, so the spectrum advantage is unquantified in outcome terms.
- It is still an oral antibiotic, and a narrower spectrum reduces rather than removes the stewardship problem.
- Nothing is known about efficacy past twelve weeks — including whether acne returns.
- The two trials' placebo arms differ by nearly five points, which widens the uncertainty on the effect.
- Tetracycline-class contraindications apply: pregnancy, breastfeeding, and children under eight.

## Suits

- Someone with moderate to severe inflammatory acne that topicals alone have not controlled.
- Someone who needs an oral antibiotic and wants the narrowest-spectrum option available.
- Someone whose prescriber will set and keep a twelve-week end date.
- Someone with coverage, without which the price is very hard to justify.

## Avoid if

- You are pregnant, planning pregnancy or breastfeeding.
- You want a treatment you can stay on indefinitely — the label caps the evidence at twelve weeks.
- You are paying cash and generic doxycycline is available to you.
- Your acne is predominantly comedonal, where a retinoid does more than any antibiotic.
- You have not been given a plan for what happens when the course ends.

## The full assessment

### What it is

Sarecycline, a narrow-spectrum tetracycline-class antibacterial, taken once daily as a 60, 100 or 150 mg tablet according to body weight, approved for the inflammatory lesions of non-nodular moderate to severe acne vulgaris from age nine. Around $262 for thirty tablets with a discount card.

### The second answer to the antibiotic problem

This register keeps finding antibiotics dispensed without a duration: an undosed oral antibiotic at R-24, topical clindamycin and metronidazole on monthly supply at R-45, topical metronidazole for 'everyday use' at R-33. R-48 showed the pharmaceutical answer — take the dose below the threshold at which the drug is antibacterial at all, and prove it with an eighteen-month flora study.

Seysara takes the other route. It stays a real antibiotic and narrows the spectrum instead, with reduced activity against the enteric Gram-negative organisms that broad-spectrum tetracyclines disturb. Then it writes the boundaries into the label.

> Efficacy of SEYSARA beyond 12 weeks and safety beyond 12 months have not been established. SEYSARA has not been evaluated in the treatment of infections. To reduce the development of drug-resistant bacteria as well as to maintain the effectiveness of other antibacterial drugs, SEYSARA should be used only as indicated.

> — FDA-approved labelling, Limitations of Use

> **THAT PARAGRAPH IS A STOP DATE** — Efficacy beyond twelve weeks has not been established. A prescriber reading that has been told, by the manufacturer, that there is no evidence supporting week thirteen. Set it against the compounded services in this register that ship topical antibiotics every month indefinitely and state no duration anywhere. The difference is not that one company is more careful — it is that one of them had to file a dossier and answer the question.

### What it does

**FIG. 1 — IGA SUCCESS AT WEEK 12** — Success required clear or almost clear with a two-point decrease from baseline. Study 1 (NCT02320149): N=483 against 485. Study 2 (NCT02322866): N=519 against 515. Note the placebo arms — they differ by nearly five points on the same protocol.

| Item | Value (% IGA SUCCESS) | Note |
| --- | --- | --- |
| SEYSARA — STUDY 1 | 21.9 | Against 10.5% on placebo: an 11.4-point gap |
| PLACEBO — STUDY 1 | 10.5 | Tablet alone |
| SEYSARA — STUDY 2 | 22.6 | Against 15.3% on placebo: a 7.3-point gap |
| PLACEBO — STUDY 2 | 15.3 | Tablet alone — nearly five points above study 1 |

_Scale maximum: 25% IGA SUCCESS._

On the co-primary lesion endpoint, inflammatory lesions fell by 15.3 and 15.5 in absolute terms — 52.2% and 50.8% — against 10.2 and 11.1, or 35.2% and 36.4%, on placebo. The drug adds roughly fifteen percentage points of lesion reduction over a placebo tablet.

The demographic table deserves a note, because this register has complained about them repeatedly. Fifty-one per cent of subjects were adults aged 18 to 45, 57% were female, and 15% were Black or African American. Against Soolantra's 96% Caucasian and Mirvaso's 99%, that is a meaningfully better-composed programme.

### The comparison nobody ran

The case for sarecycline is that a narrower spectrum means less collateral damage to the microbiome and less selection pressure for resistance than doxycycline or minocycline. That is a sound pharmacological argument and it is the entire reason the molecule was developed.

It is also untested against those drugs. Both registration trials compared sarecycline with placebo. No trial in the dossier shows that it works as well as doxycycline, or that its microbiome advantage translates into anything a patient experiences. So a prescriber choosing it is choosing a mechanism-level argument at roughly ten to fifty times the price of the generic alternative, on faith that the argument holds.

> **THE SAME GAP AS R-37** — Arazlo was never compared with tretinoin. Seysara was never compared with doxycycline. Both are plausible improvements on an incumbent, both are priced as though the improvement is established, and in both cases the registration programme answered a question — is this better than nothing — that no prescriber was asking. Vehicle-controlled and placebo-controlled trials establish that a drug works. They do not establish that it is worth choosing.

### If you are considering it

- Ask what happens at week twelve. The label says there is no evidence past it, so there should be a plan.
- Ask what generic doxycycline would cost you, and what the argument is for paying more.
- Pair it with a topical retinoid. Oral antibiotics for acne are conventionally combined, not used alone, and the combination is what limits the course length.
- Take it with enough water and stay upright afterwards — oesophageal irritation is a class effect.
- Sunscreen daily. Tetracyclines photosensitise.
- Tell your prescriber if you are pregnant, planning pregnancy or breastfeeding.
- If you are prescribed it a second time, ask whether the underlying plan is working.

### Where we stand

At 74 this is a well-documented drug doing a modest amount of work at an unreasonable price. What earns it the score is the disclosure: four limitations of use, both trials published with their placebo arms, a demographic table that stands up, and an explicit statement that the evidence ends at twelve weeks.

Its real value to this register is as a second data point. Two manufacturers faced the same problem — how do you sell an antibiotic responsibly — and gave two different, defensible answers: take the dose below the antibacterial threshold, or narrow the spectrum and cap the course. Both wrote the limits into the label. Neither telehealth service dispensing antibiotics monthly in this register has done either.
