The brand verdict

One product assessed, so this is a note rather than a pattern. Winlevi is clascoterone 1%, the first topical androgen receptor inhibitor approved for acne, and the only way to act on the androgen pathway in skin without a systemic anti-androgen. For adult women with jawline acne who cannot or will not take spironolactone, that is a real and recent addition.

The tolerability result is the strongest we have recorded against a vehicle arm. Across eight measured local reaction types, none was meaningfully worse on drug than on vehicle, and erythema and skin atrophy were both lower on drug. In a category where every alternative is a controlled burn, a treatment that irritates no more than its own base is a clinical asset in its own right.

The disclosure is unusually candid in three places. The label states that the mechanism of action for acne is unknown. It publishes HPA-axis suppression rates by age group — 5% of adults and 9% of adolescents at day 14 — with the four-week recovery interval. And it publishes the trial demographics, mean age 19.7 and 91% Caucasian, which quietly undercut the adult-female positioning the product is marketed on.

The economics are the problem, and they are stark. Around $509 discounted and $754 at retail for a 60 g tube, refrigerated before dispensing, with an instruction to discard one month after first opening. For most users that means throwing away a substantial fraction of an expensive tube on the label's own instruction. Copay assistance and coverage change the picture entirely, which is itself a reason the sticker price means nothing without checking.
